This report presented the initial experience of Cardiology Department at Cairo University with balloon catheter dilation of valvular pulmonarystenosis. Theprocedure was performed under local anesthesia in four female patients aged ten- twenty-two years. Single or double balloon catheters were used for dilatation and the technique of valvuloplasty was described in details. A reduction of systolic pressure gradient was obtained in every case; this was most prominent in cases number three and four where the gradient dropped from 110 to 30 mmHg and from 80 to 20 mmHg, respectively. The use of balloon sized 30-40% larger than the valve annulus diameter was associated with better hemodynamic results. One patient developed correctable hypotension two hours after the procedure, but there were no other complications. Balloon valvuloplasty is an effective and safe means for treatment of valvular pulmonary stenosis. Its substitution for surgical treatment will depend on the demonstration of its long-term salutary results
